Case studies

I want to try something a little different occassionally: case studies.  What I'll do from time to time is to post the details of an interesting patient I've seen (identifying details withheld, of course) and we'll work through it together.  If an interesting topic comes up in doing so, we can use the case as a launch pad to discuss it.
